If you are looking to book a new appointment, please select the appropriate location, either in office or via telemedicine (web based). If you are returning patient, you can select "Manage Appointments" to the right to schedule the right appointment for you.

You will not be charged or billed for your appointment at time of booking. Your card will be automatically charged in the case of missed visits or late cancellations per our cancellation policy.

We accept cash, check, and all major credit cards (Visa, Mastercard, American Express, and Discover). Payment is due in full at the time of visit. Insurance does not cover our services, HSA is often accepted.
